You can fill out a Virtual Authorization Card to become a dues-paying member of Teamsters Local 804 online.

- Step 1: Fill out the Form: Click "Next" and proceed to fill out all the fields on the web form.
- Step 2: Agree to Electronic Signature: Docusign will ask you to "agree to use electronic records and signatures." Click the checkbox to agree and click "Continue."
- Step 3: Add Your Signature: Docusign will take you to a PDF version of the form with your information filled out.
- Scroll down: You will see a purple "Sign" button next to "Signature of Applicant". Click this button to add your signature. Add your signature again on Page 2.
- Step 4: Click "Finish".
