Local 804 members at UPS are eligible for two weeks (10 days) of paid leave benefits if you are under a physician’s care for COVID-19 or have been advised by a physician or state agency to self-quarantine.

  • Paid leave through New York State Paid Family Leave

  • Short-term disability through the Local 804 Health Fund or TeamCare

Here are more details on these important rights.

Paid Leave for COVID-19

UPS will pay up to two weeks (10 days) of fully-paid leave to Teamsters who are diagnosed with COVID-19, living with someone with COVID-19, or have been quarantined by a government agency, medical professional or UPS.

Full-time employees will receive eight hours per day. Part-time employees will receive 3.5 hours per day.

When the Two Weeks is Exhausted

After you have exhausted your ten days of fully-paid leave, you may used accumulated paid time off or you can opt to use either NYS Paid Family Leave or short-term disability benefits if you are eligible.

If you self-quarantine and use paid time off (PTO) and are later diagnosed with COVID-19, the company will re-populate the Paid-Time-Off up to the 80/35 hours.

You cannot receive Paid Family Leave or short-term disability at the same time you are receiving other compensation from the company (paid leave or other Paid Time Off). 

New York State Paid Family Leave

Local 804 members can apply for New York State Paid Family Leave to care for a family member.

Paid Family Leave can also used by an eligible employee if they, or their minor dependent child, are subject to an order of mandatory or precautionary quarantine or isolation issued by the state of New York, the Department of Health, local board of health, or any government entity duly authorized to issue such order due to COVID-19.

New York State Paid Family Leave pays eligible workers 60% of their regular pay up to a maximum of $847.70 for up to ten weeks.

Full-time employees must have 26 consecutive weeks of employment to be eligible. 

Part-time employees who work a regular schedule of less than 20 hours per week are eligible after working 175 days, which do not need to be consecutive. 

To apply for New York State Paid Family Leave for COVID-19, fill out the COVID-19 Request for Paid Leave form and follow the instructions on the cover page. UPS requires you to call Hartford/Aetna at 866-825-0186 to report your leave. Once an application is completed, it takes 18 days for it to be processed.

Short-Term Disability Benefits for UPS Full-Timers

UPS full-timers are eligible for short-term disability benefits through the Local 804 Fund if:

  • You are under a physician’s care for COVID-19.

  • You been advised by a physician or state agency to self-quarantine. 

Short-term disability benefits are $170 a week. 

To apply for short-term disability benefits:

  • UPS requires you to call Aetna at 866-825-0186 to report your leave. Make sure to clearly state that you are only notifying Aetna of your Short-Term Disability Leave. 

  • Fill out the Disability Claim Form from the Local 804 Health & Welfare Fund. The member completes the first page (Part A), The doctor completes the second page (Part B).
  • Fax (718-786-6176) or mail the completed documents to Local 804 Welfare Trust Fund at 34-21 Review Avenue, Long Island City, NY 11101.

Within two to three business weeks you should receive your 1st check in the mail (USPS) from Amalgamated Life followed by a check each week thereafter.

Short-Term Disability Benefits for UPS Part-Timers

UPS part-timers who are covered by Teamcare are eligible for short-term disability benefits if:

  • You are under a physician’s care for COVID-19.

  • You been advised by a physician, teladoc or state agency to self-quarantine. 

TeamCare short-term disability benefits provide 60 percent of average weekly base pay up to $500 per week for up to 26 weeks, and includes continued health benefits.

Short-term disability benefits will not be paid at the same time that you are receiving paid leave or other compensation.
