What to Do If You Feel Sick
Contact your physician or a healthcare professional to evaluate your symptoms and determine the appropriate next steps.
If you are diagnosed with COVID-19 or directed by a medical professional to self-quarantine, UPS will pay you up to two weeks (10 days) of paid leave. Click here for details.
Testing
If COVID-19 testing is recommended, the Local 804 Health Fund will cover medically necessary diagnostic tests that are consistent with CDC guidance related to COVID-19 and in accordance with the federal mandate.
Treatment
The Local 804 Health Fund will cover all medically necessary care and treatment under the specific benefit levels of your plan.
To control your costs, it is extremely important to make sure that you stay in-network, including when seeking emergency assistance.
Contacting the Health Fund
We are working hard and committed to responding to all urgent requests. To limit COVID-19 from spreading, the Fund offices are operating both remotely and with limited in-person staffing.
If you have an urgent request, the best method of contacting the fund is by fax. (To comply with HIPPA, we are unable to accept emails.)
There are many free apps like FAX App that allow you to fax documents from your smart phone.
The Fund fax number is 718-786-6176. This number is forwarded to email and the box can never be full.
